This is the nomination form for USATF Masters Track & Field (MTF) Hall of Fame (HoF). Nominations are accepted all year but must be submitted before September 1st to be considered for this year. Anyone can nominate an athlete or administrator and a combination of both who has a minimum of 10 years involvement with Masters Track & Field.
Athletes must have achieved at least two (2) of the following:

• Set Individual WMA Record (even if the record has since been broken it would still be eligible for Hall of Fame consideration);
• Set Individual USATF MTF Record (even if the record has since been broken it would still be eligible for Hall of Fame consideration);
• Earned Individual World Championship (gold medal in non-relay event at a WMA Indoor or Outdoor Championship Meet);
• Earned Individual World Medal (silver or bronze in non-relay event at a WMA Indoor or Outdoor Championship Meet);
• Earned Individual National Title (gold medal in non-relay event at any USATF Masters National Indoor or Outdoor Championship meet);
• Received Athlete of the Year, including Event or Age Group, award from USATF Masters Track and Field.

Administrators include anyone who has dedicated many years to Masters Track and Field in the area of Meet Director, Administrator, Association and/or Regional Coordinators, or otherwise have made a significant impact on the progress and function of Masters Track and Field.

Anyone receiving USADA, WADA, USATF or WMA sanction or violated USATF’s Code of Conduct is not eligible.

The David Pain Outstanding Service Award is presented annually to an individual who has exhibited exceptional support and service to USATF Masters Track and Field.

The Ken Weinbel Award is presented annually in honor of Ken Weinbel (1928-2012). Ken served the masters community for many years as an athlete, meet director and administrator. He established the USA Masters Weight & Superweight Championships and promoted the Ultra Weight Pentathlon. He was meet director of those events in Seattle from their inception until 2008. He served as Chair of the USATF Masters Committee from 1996 to 2000. It is presented each year to an American masters thrower who:

1. Has made exemplary contributions to the masters throwing community over a period of years;
2. Has a strong record of accomplishment both as a thrower and as a meet organizer and administrator.

The Carol Finsrud Award is presented annually in honor of Carol Finsrud (1957-). Carol is an outstanding masters throwers with numerous World and American Records and championships to her credit. It is presented each year to an outstanding American masters thrower who:

1. Exhibits consistent excellence in performance (world and American records and championships), not just in the year in which the award is presented but over time;
2. Consistently encourages and assists fellow competitors;
3. Sets a standard both in throwing and in sportsmanship that others seek to emulate.
